Please create an online profile to verify that a license number has been issued. You may print a copy of your license from your profile once you see a license number. Please do not reapply - there are no refunds.WATCH VIDEO: Applying for a Full Exam/Score Transfer Pharmacist License Full ExamFull Exam applicants will be taking both the Naplex exam and the Arizona MPJE exam for Arizona. It is not necessary to take these exams in Arizona.To apply for an Arizona pharmacist license by full exam, you must have the following qualifications: You must be a recent graduate of an accredited US university Pharmacist degree program.If you are a graduate of a foreign university, you must submit a copy of your FPGEC certificate. You must also have earned 1500 intern hours in the US.You must also be a US citizen or a legal resident with permission to work in the United States and must meet all current US immigration policies. Score TransferScore Transfer applicants will have selected to transfer the score of the Naplex exam taken in their home state to Arizona and will only take the Arizona MPJE exam. You must have selected Arizona as score transfer state at the time of Naplex registration or within 90 days of taking the exam. If you did not select Arizona as a score transfer state you must apply by reciprocity. You can check the status of your application online here. ReciprocityThis application is for Pharmacists who have a primary license earned by Naplex exam in another state. It is also for recent graduate Pharmacists who did not score transfer their Naplex score to Arizona within the 90 day transfer period.To apply for an Arizona pharmacist license by reciprocity, you must have the following qualifications:You must be a US citizen, permanent resident (green card), or have a valid US passport. Non-immigrant visa holders (H1-B etc.) should contact the Board before applying for licensure.The license to be reciprocated must have been obtained by examination (a primary license by Naplex exam) and must be open in good standing in your original state of licensure. If it is not current and in good standing at this time DO NOT APPLY FOR LICENSURE. Please contact the state where you hold your primary license and find out what needs to be done to bring it current.Foreign graduates must have the following requirements: FPGEC Certification. You must send in a photocopy of your FPGEC certificate for validation purposes.
