Applicants holding current certification as a Registered Dietitian by the Academy of Nutrition and Dietetics may obtain Connecticut certification by arranging for the submission of the following documents directly to this office from the source (please select this link if you are not certified by the Academy of Nutrition and Dietetics): Written verification of registration sent directly from the CDR to this office; Please include CDR Registration number on your application** If applicable, verification of all licenses or certifications held, current or expired, from each state in which the applicant is or was licensed as a dietitian/nutritionist.
