For workers' compensation claims in Oregon, a nurse practitioner must become authorized prior to treating a patient. To become an authorized nurse practitioner, they MUST: -Be an Oregon licensed nurse practioner -Review the packet of information -Return the completed Nurse Practitioner Statement form (Form 440-2882, which is located on the back page of the Nurse Practitioner's Guide to Oregon On-the-Job Injuries or available online) Once the statement form is submitted, the nurse practitioner should review our List of certified care providers to ensure your information is correct.
